STEP 01

Get the prerequisites before going to the Underworld

Three things are required before you can mine Hellstone efficiently. First, a Nightmare or Deathbringer Pickaxe — crafted from Demonite or Crimtane Bars after defeating the Eye of Cthulhu or Eater of Worlds/Brain of Cthulhu. Second, an Obsidian Skull (20 Obsidian at Work Bench) to prevent fire damage. Third, access to the Underworld itself — dig a straight vertical Hellevator or find a natural path down. The Underworld is the deepest layer of the world — below all cave systems and directly above the map boundary. Equip your Obsidian Skull accessory

Before touching any Hellstone, make sure your Obsidian Skull is equipped in an accessory slot — not just in your inventory. Holding it in inventory does nothing. With it equipped, walking on or next to Hellstone blocks deals no fire damage. Without it, every Hellstone block adjacent to you deals 20 damage per second — which adds up fast when you are surrounded by a vein of ore. This is the single most important preparation step for Hellstone mining and is trivially cheap to craft (20 Obsidian).

Mine it with your Nightmare or Deathbringer Pickaxe (normal mining speed). Watch out for lava beneath Hellstone deposits — mining blocks above a lava pool can cause lava to flow and fill your work area. Use a Bucket of water to solidify lava into Obsidian before mining nearby deposits, which also gives you the Obsidian you need for smelting.

Find a Hellforge and smelt Hellstone Bars

The regular Furnace cannot smelt Hellstone — you need a Hellforge, found inside the small brick buildings scattered across the Underworld. Walk into these houses, pick up the Hellforge with your Nightmare Pickaxe, and bring it back to your base. Once placed, stand next to it and smelt: 3 Hellstone + 1 Obsidian = 1 Hellstone Bar. The Obsidian is easy to make in the Underworld itself by pouring water over any lava pool. A single Hellforge serves for the entire game and doubles as a regular Furnace too.
