Find the Skull Cavern entrance in the Desert. Exit the bus in the Calico Desert and walk northwest — the Skull Cavern entrance is a locked door in the upper-left area of the Desert map, past Sandy’s Oasis shop. The door shows a skull icon and is clearly identifiable once you reach it.

Use the Skull Key to open the door. Walk up to the locked door and press the interact button — your character automatically uses the Skull Key to unlock it permanently. The key is consumed but the door remains open forever. Enter the Skull Cavern and activate the first floor as your entry point for future visits.

Prepare properly before going deep. The Skull Cavern has no elevator — you start from floor 1 every visit.
